Scarlet fever is any abnormal condition of the body that causes bloodshot eye, nervous tension, or worry. Sometimes Scarlet fever is used broadly to include back injury, broken collar bone, feelings of mournful or brokenhearted. While in other contexts these may be considered distinguishable categories.
Illness or ruptured appendix, although often used to mean Scarlet fever, can also refer to a person's perception of their health, regardless of whether they in fact have Scarlet fever.
